And a boiler sized for the old house cannot heat the new one.
Coombe and Kingston Hill hold the best stock in the borough: large detached houses, most with a cylinder, many with two zones. Surbiton is Victorian villas and mansion flats, and the town centre end of KT1 is flats and newer conversions. What runs across all of it is extension. Rear extensions, side returns, loft conversions and glazed garden rooms have been going in here for twenty years.
The heating rarely grew with the house. Radiators get added to a system whose boiler, pump and pipework were specified for the original floor area. In mild weather nothing shows. In a genuine cold snap the house never quite gets warm, the radiators run warm rather than hot, and the boiler runs almost continuously trying to make up a deficit it cannot.
That is a sizing and balancing problem, not a fault, and it is the one most often answered with a new boiler. A larger appliance on undersized pipework changes very little. Measuring the actual demand, rebalancing the system and checking the pump against it costs a fraction of a replacement and usually settles it.
A glazed garden room loses heat faster than any other part of a house, and it is almost always the last thing added to the heating. If one room is permanently cold and it is the one with the most glass, the boiler is not the problem.
The commonest call from KT2, and it is rarely the boiler.
On a stored system, heating that works and hot water that does not points at the cylinder side. Three usual culprits: the motorised valve that diverts flow to the tank, the cylinder thermostat, and the immersion element. Each costs a fraction of a new appliance and each gets mistaken for one.
In an extended house there is a fourth possibility worth ruling out: the hot water is losing the competition. Where a system is at its limit the heating zones can take everything the boiler produces, leaving the cylinder to recover only when they are satisfied. The symptom looks like a failing cylinder and is really a demand problem.
An unvented cylinder is a pressurised store, so working on one is notifiable and needs a G3 qualification on top of Gas Safe registration. The one thing a yearly visit genuinely prevents.
London water comes off the Thames and the Lea, both of which run through chalk. It measures roughly 277 milligrams of calcium carbonate per litre, and that figure is much the same across the city.
Where it lands is what differs. In a combi it furs the plate heat exchanger and the hot water goes lukewarm. In a Coombe or Kingston Hill house that kept its cylinder it lands on the coil inside the tank. In an extended system it does something else as well: more pipework and more radiators means more places for magnetite to settle, which is why a magnetic filter earns its keep faster on a big system than a small one.

Age does not decide it. The failed part does.
A pump, a diverter valve, a thermostat or a fan is a repair on almost any appliance, whatever its age. A failed main heat exchanger on a boiler well past ten years is the point where the arithmetic stops working, and we will tell you that rather than sell you the repair.
In an extended house the replacement question is the wrong question until one thing is measured: whether the system, not the appliance, is at its limit. A bigger boiler on pipework and a pump sized for the original house will run harder and heat no better. That measurement takes an hour and it decides whether a replacement is worth anything at all.

House never quite gets warm?
If the radiators are warm rather than hot and the boiler runs constantly, that is usually a system that has grown past what it was sized for. It is measurable, and it is not a new boiler.
If you smell gas: do not touch any switches. Open the windows, leave the property, then call the National Gas Emergency Service on 0800 111 999. That line is free and open 24 hours. Call them before you call us.
If a carbon monoxide alarm is sounding: get everyone out immediately, then call 999 if anyone feels unwell. Do not go back in until the property has been ventilated and checked.
If the boiler locks out repeatedly: try one reset. If it locks out again straight away, stop. Repeated resets hide a fault that is still there, and on a gas appliance that is not a risk worth taking.
Under the Gas Safety (Installation and Use) Regulations 1998, only a Gas Safe registered engineer may work on gas appliances. Never open the boiler casing yourself.
